The Ohio Medical Marijuana Control Program is temporarily allowing doctors to meet with patients online through telehealth to assess eligibility for a medical marijuana card prescription.



Ohio lawmakers passed House Bill 523 in the Summer of 2016, making medical marijuana legal in Ohio, and established the rules governing the Ohio Medical Marijuana Control Program. The Ohio Medical Board has approved the use of medical marijuana to help treat and alleviate the symptoms associated with a variety of medical conditions and ailments. At Ohio Restorative Medicine, we recognize the overwhelming evidence supporting medical marijuana's potential to improve one's quality of life, and after a comprehensive evaluation will provide recommendations as warranted.


Pre-Screening Paperwork: Print, complete, and submit pre-screening paperwork. Not everyone is eligible for a medical marijuana recommendation. You MUST MEET ELIGIBILITY REQUIREMENTS, or we will not provide you with a recommendation. We will review your paperwork and contact you to set up an appointment or to inform you that you do not meet eligibility requirements and suggest other alternatives.

Appointment: Appointments can currently be performed through telehealth. You must have a prior qualifying medical diagnosis from a physician and HAVE MEDICAL RECORDS verifying the qualifying condition(s) in addition to a valid form of State I.D. With a signed release (part of the pre-screening paperwork) we may be able to access your medical records electronically. The cost for an initial medical marijuana evaluation is $250.

Recommendation & Registration: If you are determined to qualify for a medical marijuana recommendation, our physician will provide a recommendation to prescribe to the Ohio Pharmacy Board and you will be added to the Ohio Medical Marijuana Control Program Registry. Immediately after being added to the registry, you will receive an email with a registration link that is good for 90 days and can only be accessed once. The annual registration fee for the Patient Registry is $50. The Board of Pharmacy may reduce registration fee by 50% for a patient who qualifies for disabilities, social security, or supplemental income; or veteran status.

Dispensing: After completing your registration, you will be able to print off your official Ohio Marijuana Card. Bring your card to a dispensary and purchase your medical marijuana product.

Renewal: Your card is good for one year. You will need to meet with our physician yearly to renew it. The cost for a follow-up medical marijuana consultation is $150.


What are the qualifying conditions to receive medical marijuana in Ohio?

Amyotrophic lateral sclerosis (ALS)
Alzheimer’s disease
Cachexia, Wasting syndrome
Chronic traumatic encephalopathy
Crohn’s disease
Epilepsy or another seizure disorder
Hepatitis C
Inflammatory bowel disease
Multiple sclerosis
Pain that is either chronic and severe or intractable
Parkinson’s disease
Post-traumatic stress disorder
Sickle cell anemia
Spinal cord disease or injury
Tourette’s syndrome
Traumatic brain injury
Ulcerative colitis

How soon can I get an appointment? It is possible to schedule same day appointments.

How soon can I visit a dispensary after being added to the registry? As soon as you complete your registration, you may visit a dispensary. Thus, if your appointment is early enough, you may be able to visit a dispensary the same day as your appointment.
